Support annotations for license terms

For each license term value, user can add a term-specific annotation
NB "annotation" was what were previously labelled "notes" in wireframes - see attached w/f.

• One optional annotation per term value
• Annotation is always shown, even if blank (to indicate that no annotation has been set)
• Appears in
  • Licenses UI
  • Agreements/Controlling License Terms UI
• Up to ~paragraph length
• In a specific license context, the term must be set (as opposed to Not Set, eg, for default terms) for it to be able to have an annotation (ie, no orphan annotation)

Out of scope:

• No history, search, title or categorisation (see UXPROD-1511)
