Overdue loans report (additional loan fields)

Purpose: Include loan identifier & human-readable loan policy name on overdue loans report.
Story: As a circulation staff member who handles billing, I want to know which loans are overdue and what policy applies to them, so I can assess the severity of the fine needed and be sure I'm adding the fine to the right loan.
