Pay and Waive FF fixes for double ":" and "*"

Formerly known as Pay and Waive FF fixes for double ":" and "*" plus missing "for staff"

While testing patron notices at http://folio-q1-2019-core.aws.indexdata.com, Holly came across this issue. UIU-959 is for a similar issue with the PATRON BLOCK pop-up.

The PAY and WAIVE modals both have these issues:

• The end of three fields have double colons :: instead of a single colon : FIXED AND TESTED!
• The required amount field has double asterisk ** instead of a single asterisk * FIXED AND TESTED!
• "Additional information" should have been renamed to "Additional information for staff" THIS FIX HAS BEEN MOVED TO UIU-967
