Users: Permission Set Created Twice

    Description

      Steps taken (can't repro):

      1. Logged into https://bugfest.folio.ebsco.com/ as folio/folio
      2. Went to Settings > Users > Permission sets
      3. Created a permission set called "Custom: All permissions", gave it a description then painstakingly added every single permission to this set
      4. Clicked save and close

      Expected: One permission set should be created

      Actual: Two were created

      Additional information: I have since deleted the duplicate. Could not reproduce.
