Charge and Pay Fee/Fine "Notify Patron" options not fully implemented

Notifications are not working right now so nothing is generated for the patron!

1. "Charge Manual Fee/Fine" and "Pay Fee/Fine" have were updated in Q1 2019 to be able to create patron notices (see UIU-710, UIU-711 and UIU-713) but they are not actually being generated and sent out yet.
2. "Waive Fee/Fine" and "Cancel Fee/Fine" have a default setting of *Notify Patron" (which can be de-selected), but nothing else has been done.
   • Need to rename Additional information to Additional information for staff
   • Need to add Additional information for patron (if Notify patron box is checked)
