"New Fee/Fine" modal: If notice exists need to display "Notify patron" checkbox and "Additional information for patron"

Important note: This work needs to be completed by June 27 so Holly can test it and release it to Morning Glory BugFest by July 4.

Overview
The modal to replace the "New Fee/Fine" page is in place, but it needs to have two more fields displayed if a notice is set up to be sent to a patron...

Scenarios

1. Scenario
   Given New Fee/Fine modal open
   When Fee/Fine Owner selected that has a "Default charge notice"
   Then update the New Fee/Fine modal to add Notify patron and Additional information for patron, as shown in attached mockup Modal-New-FF-with-notice.jpg)
2. Scenario
   Given New Fee/Fine modal open
   When Fee/Fine Type selected that has a "Charge notice"
   Then update the New Fee/Fine modal to add Notify patron and Additional information for patron, as shown in attached mockup Modal-New-FF-with-notice.jpg)
3. Scenario
   Given New Fee/Fine modal open with _Notify patron" box checked
   When user unchecks Notify patron
   Then display the Notify patron option unchecked and remove Additional information for patron
4. Scenario
   Given New Fee/Fine modal open with _Notify patron" box unchecked
   When user re-checks Notify patron
   Then display the Notify patron option checked and add Additional information for patron back
