Create user defaults wrong (Status = inactive, Preferred contact = Select contact type)

Background: I am 99% sure that these defaults used to be different

Steps to repro:

1. Log into folio-snapshot
2. Go to Users and create a new user

Expected:

• Default status for new users should be Active
• Default preferred contact type should be email

Actual:

• Status is inactive by default
• Preferred contact is unselected by default

This makes it difficult to create a new user and it's easy to not realize your new user is inactive until you try to do something with them.

Additional info: Subsequent edits of the user record (to, say, add a number to the barcode) will automatically change the user status to active
