Finish implementing patron notices for fee/fine cancellations

When patron notices were implemented for new fee/fine and pay fee/fine, they were also implemented for waive fee/fine, transfer fee/fine, and cancel fee/fine, except that the modals were not updated appropriately. Additionally, the Notify patron flag needs to be set by default in certain cases. Right now it is always checked.

The purpose of this JIRA issue is to finish the implementation of the patron notice when a fee/fine has been cancelled in error.

Change "Additional information" must be changed to "Additional information for staff" as shown in all three attachment mock-ups.

When the user invokes the "Cancel fee/fine" modal, one of the following 3 options will apply...
1. If there IS an "Action notice" associated with the "Fee/Fine Type" in the Manual Charges table, display the "Notify patron" option as on and display the optional "Additional information for patron" box (as shown in attached screen mock-up Notify-Patron-Error-Modal.JPG).
2. If there IS NOT an "Action notice" associated with the "Fee/Fine Type" in the Manual Charges table, but there IS a "Default action notice" set for the "Fee/Fine Owner", display the "Notify patron" option as on and display the optional "Additional information for patron" box (as shown in attached screen mock-up Notify-Patron-Error-Modal.JPG).
3. If there IS NOT an "Action notice" associated with the "Fee/Fine Type" and there IS NOT a "Default action notice" set for the "Fee/Fine Owner", do not display the "Notify patron" option or optional "Additional information for patron" box (as shown in attached screen mock-up Notify-Patron-Error-Modal3.JPG).

If the user de-selects the "Notify patron" option, display screen as show in attached mock-up Notify-Patron-Error-Modal2.JPG.
