Can't edit fields on create and edit views of service point form

Steps:

1. Log into folio-testing as diku_admin
2. Go to Settings > Tenant > Service points
3. Click New to create a new SP
4. Attempt to edit the fields on the form

Expected: Should be editable

Actual: Not editable
