TLR: when changing from title request to item request, form can't "save & close"

    Description

      Overview: When placing a Title level request and some of the form information is populated (title, requester) and the Request is changed to an item level request by deselecting the "Create title level request" box, the form is unable to Save & close
      Steps to Reproduce:

      1. Log into snapshot
      2. Open a new Title level request form
      3. Find a title and requester
      4. Select a Pickup service point
      5. Deselect the "Create title level request" box
      6. Select an item 
      7. Select Save & Close

      Expected Results: The request is saved and closed, the user is directed to Request details
      Actual Results: Nothing happens - the page hangs
