When duplicating a request, the pickup location is not preserved

Overview:

When duplicating a request in Lotus Bugfest, the original pickup location selection is not preserved. It appears selected briefly, but then the menu is reset to "select a pickup location".

Steps to Reproduce:

1. Log into Lotus Bugfest
2. Find an existing closed request
3. Select the request detail action menu > Duplicate

Expected Results:

The create request form should be shown with all fields populated based on the selections in the original request, including pickup location.

Actual Results:

The pickup location selection is not set.
