Unchecking the Title level request box when it is automatically checked

Purpose: To describe what should happen when a user makes a title level request because the setting for "Create title level request" selected by default has been enabled and the user unchecks the Create title level request box at the top of the request form

User story statement(s):

As library management,
I want Circulation staff to be able to change a new, unsaved title level request to a new, unsaved item level request

Scenarios:

1. Scenario:
   • Given Request permissions https://docs.google.com/spreadsheets/d/1IJnlWvi4k-wFatDNimx9fQ375evHSIIxtGMookHvs-I/edit?usp=sharing 
   • When I have the permission
     • Requests: View, create
     • Requests: All permissions
     • Requests: View, edit, cancel
   • Then the following scenario (3) is true
2. Scenario:
   • Given Request permissions https://docs.google.com/spreadsheets/d/1IJnlWvi4k-wFatDNimx9fQ375evHSIIxtGMookHvs-I/edit?usp=sharing 
   • When I do NOT have the permission
     • Requests: View, create
     • Requests: All permissions
     • Requests: View, edit, cancel
   • Then I am unable to complete the action in the scenario below
3.  Scenario:
   • Given a request with the Create title level request check box selected 
   • When I deselect or uncheck that box
   • Then the fields will change from instance fields to item fields (UIIN-1619) AND show the available items on the instance (that had been selected) so that an item can be selected

Note: this is like the move request dialog box already in the requests app - in this case, the list would just include ALL the items associated with the selected instance.
