Update (new) request form

      Purpose: To update the request form to include title level requests and title lookup

      User story statement(s):

      As a library staff member,
      I want to be able to create a new item level OR title level request from the same form


      1. Scenario:
        • Given Settings > Circulation > Requests
        • When the checkbox for "Allow title level requests" is selected but the parameter for "Create title level request" selected by default is NOT selected
        • Then the following scenarios are true

        • Given the Action menu from any of the following apps:
          • Requests
          • Users
          • Inventory
        • When I select New request or New
        • Then an updated request page/form loads with a new check box option at the top "Create title level request" 
      4. Scenario:
        • Given the new request page 
        • When the Create title level check box is checked (manually or through the default setting)
        • Then the following additional updates are made to the new request page
          • First accordion = Title information 
            • Title level requests (number of requests link)
            • Title (Name of title link)
            • Contributor
            • Publication date
            • Edition
            • ISBNs (maximum 4 ISBNs should display)
      5. Scenario:
        • Given the new request page
        • When the Create title level check box is NOT checked
        • Then the new request form appears with the item level information filled in
