Request Edit Page Intermittently Missing Lots of Data

    Description

      Steps to repro:

      1. Log into folio-testing
      2. Go to Requests
      3. Click a request to view
      4. Click to edit

      Expected: Request details edit screen should look like the view page except that the two date fields at the bottom are editable

      Actual: Intermittently seeing a very weirdly formatted page missing tons of data. See attached screenshot and console output.
