Purpose: Once we have implemented request as proxy (~~UIREQ-50~~), we want to add a column to the requests list for the Proxy name

- When displayed
- Then a new Proxy column should display as shown in the below linked wireframe

- When displayed
- Then display the name of the proxy requester, if applicable:
  - <lastName>, <firstName> <middleName>
  - Don't display the comma if only the last name is present
