"Requests on item" count incorrect when creating new request

Steps:
Create two items on the same holding record.
Add two requests to item A.
Go to the requests app
Cancel one of the requests
Select New request to create new request
Enter the barcode of item A

Expected result:
When item A is found, "Requests on item" should say 1 because there is one open request.

Actual result
Says 2, presumably counting the closed request

Additional info: If you click the link and say "close without saving" on the popup, it takes you back to the request list page and nothing is showing. So that's another bug (it should show you all the open request on the item)
