Add Title for receiving/check-in

Purpose: Add title object to hold receiving and title information for one or many pieces relating to a POL

Acceptance criteria:
Given the user wants to add title for receiving
When clicking "New"
Then user arrives at create title view in full screen

Given the user wants to add title for receiving Not all data available, will test as part of schema update issue
When adding title information
Then the user can input the following:

• Title (Required, lookup or input, not repeatable)
  • Additional title information (Publisher, Pub date, Edition, Sub dates, Sub interval, Contributors, Product IDs)
• POL number (Required, lookup, not repeatable)
• Expected date (Date picker, local format, not repeatable)
• Receiving note (string, not repeatable)
• Must acknowledge receiving note (Toggle)

Given the user is adding title info
When user clicks Title look up link
Then they can select a title from instance using instance plugin
AND instance information populates Item detail fields

Given the user is adding POL number info
When user clicks POL look up link
Then they can select a POL using the POL lookup plugin

Given the user has added detail
When ready
Then they can click "Save and close" or "Cancel" to dismiss the form

Given the user has clicked save and close will create separate story for implementing message
When the have added all necessary detail
Then system displays a green toast message indicating operation was successful
