Allow user to indicate in order template that specific fields should be hidden on order form

Purpose: Allow user to indicate in order template that specific fields should be hidden when order form is used with that template. This will help user that are creating orders to focus on providing the data that administrators need them to provide and nothing else.

User story statement(s):

As an acquisitions administrator,
I want to be able to hide fields from the order form
so that users creating orders are focused on providing the information needed for that type of order
AND they can do so as quickly as possible

Scenarios:

1. Scenario:
   • Given user has permissions to create/edit order template
   • When creating/editing order templates
   • Then certain fields display an eye icon
   • 
2. Scenario:
   • Given user has permissions to create/edit order template
   • When creating/editing order templates
   • Then user can click "eye" icon to toggle show or hide field
3. Scenario:
   • Given user has permissions to create/edit order template
   • When creating/editing order templates
   • Then user can click Hide all fields toggle to set all "Hide" toggles to true
4. Scenario:
   • Given user has permissions to create/edit order template
   • When creating/editing order templates and all eligible fields are set to hide
   • Then user can click Hide all fields toggle to set all "Hide" toggles to false
