Create order templates in settings

Purpose: Order templates will speed up/simplify the process of ordering materials and limit the margin of error in ordering.

Acceptance criteria
Given the user has navigated to Order settings
When selecting "Order templates"
Then the user can add a new order template

Given the user can create a new template
When creating the template
Then the user can add default values and selections to the order form fields
AND this includes both PO and POL fields

Out of scope:
Given the user can create a new template
When creating the template
Then the user can set show or hide for all un-required fields
Given the user can create a new template
When creating the template
Then the user can set additional fields as required to submit the form
