Can not create pre-set adjustments

Overview: Cannot save preset adjustments in the settings area

How to reproduce:
Navigate to settings->Invoices->adjustments
Click new
Complete form
Click save

Actual: create form is dismissed but adjustment is not save to list. also, they can not be selected during order and invoice create/edit.

Expected: create form is dismissed and Adjustment appears in list. When editing or creating invoices or orders these adjustments appear in the "pre-set adjustments" filterable select list.
