Job profile - adding new profile

Clicking on the New button in the third pane of the Settings -> Data export -> Job profiles will open a new form that will be used to create a new job profile.

Acceptance criteria:

1.The form contains:

• Name (required field)
• Mapping profile drop-down (required). The drop-down is pre-populated with the list of already existing mapping profiles (max 50 records)
• TCP/IP drop-down ( not required) Disabled in Q2
• Description (not required)

2. Cancel button (cancel functionality covered by UIDEXP-83)
3. Save & Close button is disabled until both required fields are populated (save functionality is covered in UIEXP-85)
