Add Last Updated Record Metadata to Loan Policy Form

Purpose: To add the standard "record metadata" to the loan policy form so it is consistent with other records in FOLIO.

Use case:
As a user who is reviewing a loan policy record, I want to know who last updated it and when so that I can reach out to them if I have questions about what I see or if I am surprised that it was recently updated

Scenarios:

1. Scenario
   • Given the view mode of the loan policy form at Settings > Circulation > Loan policies
   • When displayed
   • Then the standard record metadata should appear at the top of the form (see attached screenshots of this component on other forms
2. Scenario
   • Given the edit mode of the loan policy form at Settings > Circulation > Loan policies
   • When displayed
   • Then the standard record metadata should appear at the top of the form (see attached screenshots of this component on other forms
