Add preferred name to the Proxy Modal

As a staff user,
I want to see the patron's preferred first name in the Proxy Modal
so that I might address the patron properly.

Scenario 1:
Given that a preferred first name exists in User A's record (See screenshot 1)
And Given that User A is a proxy for User B
When User A's record is retrieved in the request UI
Then verify that in the proxy modal, User A's information appears as Last name, preferred first name, middle initial if any (screenshot 3)
