      Description

      Steps to repro:

      1. Log into folio-testing
      2. Go to Settings > Tenant > Locations > Institution (or really any other page in settings using this same "controlled vocab crud" component
      3. Create an institution with duplicate name

      Expected: Should disallow save with meaningful message displaying somewhere you can see it consistent with the rest of folio. Can we have the message in red text under the field like we do elsewhere? If it's easier to implement this across the board for all controlled vocabs, we could have a generic message like "Must be unique" or something.

      Actual: Disallows save, but the message is obscure and not visible on page
