Settings module: Unsaved Changes Modal not popping up after being closed once and Save/Update button does not work

Overview

• It appears this issue happens with every New or Edit form that displays an "X" in the upper lefthand corner in every app with the exception of the eholdings app and Users app.
• The "X" and Save/Update buttons do not work. BUT the Delete button does work.

When using any form in Folio that is using the stripesForm from @folio/stripes-form you will only be prompted once when trying to exit while having a "dirty" form and it will block you from leaving the page.

Steps to reproduce:
1. Go to any edit view (e.g. Inventory -> click on instance -> click edit)
2. Type something in one of the fields to make the form "dirty"
3. Click on the X in the top left corner. You will get prompted. Click "Continue editing". The modal closes
4. Click the X again and nothing happens
5. Select Cancel editing under the pane header dropdown and nothing happens
6. Try to Save or and nothing happens.
7. Use the browser to get out of the record
8. Edit the record again and unable to do anything to the record.
9. This issue happens across all apps.

Acceptance Criteria

Given I am on a New form page
AND I add data
AND I hit "X" OR select the Cancel option in the pane header dropdown
WHEN I hit the Keep Editing option on the Unsaved Changes modal
AND I hit "X" OR select the Cancel option in the pane header dropdown again
THEN display the Unsaved Changes modal again
AND Save/Update buttons should be executable

Given I am on a Edit form page
AND I add more data
AND I hit "X" OR select the Cancel option in the pane header dropdown
WHEN I hit the Keep Editing option on the Unsaved Changes modal
AND I hit "X" OR select the Cancel option in the pane header dropdown again
THEN display the Unsaved Changes modal again
AND Save/Update buttons should be executable
