Update ui-app template with demo settings

The current ui-app template only demonstrates a /settings route with a couple sample settings groups to navigate.

Add some sample settings, such as a toggle and a string value. These settings should make use of the ConfigManager stripes-smart-component in the UI and persisted to the back-end.

Make use of the sample settings elsewhere in the application, such as on the /example page to demonstrate usage.
