Allow custom prefix when creating index name

Currently mod-search creates the index with name like instance_<tenantId>. It would be nice to allow hosting team to add a prefix through env config so it can look like <prefix>_instance_<tenantId> so the same AWS ES can be shared.

The prefix should be added for creation and query.
