Duplicate key value violates unique constraint "records_lb_pkey"

When execute data-import-flow via "Create job profile (for instance+holdings+item)", there can be some logs from mod-source-record-storage:

The main goal is to investigate the root cause of this log and fix it.

Note: this error message can be revealed after the same chunk of messages were reached to srs. 

Might relate to duplicated events
