Extend mod-notify to support sending password creation/reset/changed password emails

To support a Folio user receiving an email to set up his/her password and to support a user being informed when his/her password has changed, we need a mechanism that supports the following

• Send a Change your password email to the user when the password is successfully updated.
• Send a Create your password email to the user when the Folio staff admin triggers sending this email. (see screenshot https://drive.google.com/open?id=0By8ccf5VV4EWREVNQWJwRGphb0E)
• Send a Reset your password email to the user when the user successfully verifies that s/he is the person requesting the reset. (See screenshot https://drive.google.com/open?id=0By8ccf5VV4EWTmtZRFo4RWtHTGs)
• Send a Reset your password email to the user when the Folio staff admin triggers sending this email

Additional requirements

• Based on conversations with Heikki, it maybe best to extend mod-notify to support sending password creation/reset/changed password emails
• Implementation needs to support additional emails/notifications requiring this capability and changes to the emails noted above.
• Need to ensure we are logging date/time the email is sent and delivery status
• This requirement IS NOT to implement the Notifications App. The key requirement is to develop a mechanism/backend service(s) to send an email to a user.
