Fix structure of statistical codes, instance record and item record

Statistical codes should have been implemented in the instance and item schema as simple lists of UUIDs - not as objects containing a code type and a free text code.

Statistical codes were originally conceived as a statistical code type with a free text code, but since then, the SIG has decided that the codes should be a controlled list as well. Accidentally this change of design not been implemented as decided.
