Data Import unable to create items with Data Import profile

The following issue is observed in an Iris Hotifx-3 environment

Import profiles will not create item records though SRS, Instance, and holdings records are created. Specific error is "Barcode must be unique, null is already assigned to another item" – i.e. it is attempting to create a barcode and assign it a value of null even though nothing in Data Import profile attempts to load anything into barcode field.

Issue is observed when doing a single record OCLC import. Confirmed Import profiles are set up correctly

Note from abreaux: I have not been able to reproduce outside of this environment. It may be a configuration issue. See if anything in the logs helps with identifying the problem
