Refund report - Paid amount is 0.00 in some scenarios

Steps to reproduce:

1. Create multiple fees/fines using Charge and pay button leaving Transaction info field blank
2. Refund each of these payments fully or partially
3. Download Refunds to process manually (CSV) report

Expected result:

1. In each line Paid amount should be equal to Billed amount
2. All payments should have correct Payment method value
3. Payment Transaction information should be equal to "-"

Actual result:

1. Paid amount is "0.00" in some of the refunds
2. Payment method is blank
3. Payment Transaction information is blank
