Report upload error: "Index out of bounds"

Overview:

CSV Report cannot be uploaded, although it seems to be formatted correctly.
Steps to Reproduce:

1. Log into some FOLIO environment (Iris and current snapshot)
2. Go o eusage, some UDP
3. Upload the attached report

Expected Results:

Report is uploaded and stored.

Actual Results:

Upload fails with the following message:

Failed... Error saving report: org.folio.rest.impl.CounterReportAPI$ReportUploadException: java.lang.IndexOutOfBoundsException: Index 1 out of bounds for length 1

Additional Information:

The original report was much larger (>60MB), I shortened it for conveniant testing - the error message stays the same.
