Automatic migration of permissions during module upgrade

This ticket groups all the work required to investigate, design and implement support for migration of permission sets during module upgrade.

As a result of this work, FOLIO users and operators may expect that whenever a static permission set is modified, removed or renamed through the module's Module Descriptor a corresponding change will be applied to the permission stored in the system's permission database (mod-permissions) and any corresponding user assignments will be updated as well.

Additionally, the system will provide a way to handle (re-assign and clean up) deprecated permission sets.
