Error creating Confluence page from template

Overview:
Steps to Reproduce:

1. Log into Confluence
2. Click on the `...` next to `Create` on the orange menu bar to `Create from Template`

Expected Results:
The dialog box with choice of space templates should be presented.

Actual Results:
A dialog box with a spinning wheel is presented. No templates are shown for selection.
