Update folio-install single-server procedure for q1-2019 release

Update the folio-install/single-server.md document.

Will be similar to that done for the q4-2018 release (FOLIO-1651).

There are now more modules, so vagrant memory requirements will need to be estimated and increased.

The section about creating a "superuser and load permissions" can be modified, following FOLIO-1798.

The reference and sample data will now be loaded via the modules. The scripted technique for loading data, and the local data directories, can be removed.

The "bleeding-edge" instructions does build a list of modules. Some do not get pulled in by dependency. That list of extra modules might need to be adjusted.
