Agreements: New/Edit: Correct the card display for Organizations and Usage data providers

Organizations

When an organization is linked to an agreement the organization name is displayed in the card body. The Name field should have a label and be displayed as shown in the attached mockup. This will bring presentation in line with all other lookups (and make it possible to display other fields in the Organization card in the future).

This change affects the New/Edit license pane. The license preview is not affected.

Usage data

The same change needs to be applied to the "Usage data provider" card in the "Usage data" accordion.
