Package identifiers not 'removed' from package on ingest of updated package

Overview:
Steps to Reproduce:

1. Import simple_package_for_updates_1.json (attached)
2. See package created with identifiers (namespace: id)
   1. isil: ID1
   2. ezb: ID2
3. Import simple_package_for_updates_2.json (attached)
4. See package updated with new identifiers
   1. isil: ID3
   2. ezb: ID4
5. See old identifiers still also display

Expected Results:

Old identifiers status set to 'error' and no longer retrieved with package metadata over API

Actual Results:

old identifiers continue to be retrieved with status = approved
