UI for managing proxy server configurations

The mockup here should be used as guidance for implementation, but modified in the following way:

The list of variables should be: inputUrl, platformLocalCode
Use "Helpers" rather than "Handlebar helpers"
The list of Helpers should be: encodeUrl, insertAfter, insertAfterAll, insertBefore, insertBeforeAll, encodeUrl, removeProtocol, replace

Platforms to exclude from proxy server setting
The proxy server template will not be used to generate proxied URLs for any platforms listed here
